The knife blade is retracted in the photo. Look behind the trigger and you can see the tip of the blade is against a silver pin. This is not the classic apache duster, they have a folding set of knuckles as well as a folding trigger.

After you'd fired your 5 shots, you used the handle as brass knuckles.
(some even had a folding knife blade; people knew how to take a bite outa crime in them thar days)
